Well, aside from Romana (who was a time lord too) it is true that on screen only Jamie McCrimmon was ever played by more than one actor. In that case, it was due to illness and a need to keep to a production schedule.
While many of us here at BrokenSea only dream of being able to make a living at audio drama, Laura Post is actually doing it. You can hear Laura in many places including Tweeny Witches.
In talking with Laura, it became clear that between a day job and trying to keep up with paying VA assignments, she would not be able to keep up with the production schedule for her role of Olivia La Pluie on our Doctor Who. Though we were sad to hear this news, we wish Laura well in all her endeavors (and hopefully she will still be able to sneak in a cameo role here and there).

She can always pull a Sarah Jane and crop up from time to time. I'm assuming that's not unheard of before the new series, outside of the 5 doctors thing that I've seen. If your not dead you can still return

We are having a new actress take over the role instead of writing the character out. Since we have a plotline that involves Olivia until at least the end of 2009, we can't just drop her on a planet somewhere.

Minor throat injury if there's a difference in the voice. she looses her voice, the Doctor fixes it with his sonic screwdriver and the results are slightly worse than 100%

We're just planning on doing the switch next week — in Strange Bedfellows. Maybe we could use your idea for some levity in the April Fools episode?

Well, you'll have to explain it somehow and comedy is generally the route to take. Could always do it like the little dream thing that came out this week.
